EVALUATIONS - Substance Abuse (Drug / Alcohol)

Steps to completing AN EVALUATION

THE PROCESS

1. Complete all 3 assessments below (MAST, DAST, and SALCE).

2. BOOK (Choose date and time).

3. Make Credit Card payment.

4. Receive first email (Booking Confirmed - Your receipt).

5. Receive second email within 24 hours (Portal Sign-IN link).

3. Sign into Portal. Complete all forms/documents in portal and upload picture ID.

6. Call office (832) 830-8662. Go over paperwork and receive virtual meeting link.

7. Attend Virtual meeting (30 -45 mins) with Licensed Professional Counselor.

8. Eavluation recommendation completed by the counselor and sent to C.S.O. within 48 hrs.

2. Select - SALCE Interactive Survey (English) - for ID # use the last four of Social Security Number

3. Complete the Survey be sure to answer all questions as accurately as possible.

3. Click Submit - It will automatically come back to the therapist for review.
